Workshop Proposal Forms

If you are interested in offering a workshop/program through the Gestalt Institute of Cleveland you must complete one of the workshop proposals below.

Any workshop offering CE hours to Clinicians must complete the “Clinical Workshop Proposal”.

Any workshop offering CCE hours for Coaching must complete the “Coaching Workshop Proposal”. We will then submit to ICF for approval. Also needed is the ICF Program Schedule. That template is available below.
